ScienceDaily (Sep. 28, 2012) ? A new study reveals that cognitive therapy over the phone is just as effective as meeting face-to-face. The research was published September 28, in the journal PLoS ONE.
Researchers at the University of Cambridge together with the National Institute for Health Research Collaboration for Leadership in Applied Health Research & Care (NIHR CLAHRC) and NHS Midlands & East also found that providing talking therapy over the phone increases access to psychological therapies for people with common mental disorders and potentially saves the NHS money.
For the study, data from 39,000 patients in seven established Improving Access to Psychological Therapies (IAPT) services (an initiative which aims to expand the availability of psychological therapies) in the East of England were used to compare Cognitive Behavioural Therapy (CBT) delivered face-to-face versus over the phone. For all but an infrequent, identifiable clinical group with more severe illness, therapy over the phone was as effective as face to face, and the cost per session was 36.2% lower.
Patients may be unable to access health services due to transport problems, work commitments and physical disability, among many reasons. So increasing availability of talking therapies over the phone will make mental health services more accessible to patients.
On the back of the study results, NHS Midlands & East has instigated a regional training programme to standardise service delivery and ensure therapists are competent at phone contacts. The training programme has recently been extended into a partnership with a third party organisation.
Professor Peter Jones, Principal Investigator of the study from the University of Cambridge, said: "Providing therapy over the phone will not only help individuals gain much-needed access to mental health treatment, it will provide a more cost effective way of providing these services at a time when everyone is concerned about cutting costs."
Mental health illnesses affect one in four adults in Britain every year. Additionally, the NHS spends more on mental health than it does on cancer, heart disease, stroke and asthma put together (a total of ?9.95 billion in 2010-2011), with general practitioners spending more than a third of their time on mental health issues.

NEW FAIRFIELD, Conn. (AP) ? Tyler Giuliano had no trouble with the law. The teenager loved flying small planes as a Civil Air Patrol cadet and seemed happy as he played an online game with friends Wednesday night. But hours later, authorities say, Tyler was outside wearing a black ski mask and wielding a knife when he was shot by his father, who thought he was a prowler.
No immediate charges were brought against Jeffrey Giuliano, a popular fifth-grade teacher, in the slaying of 15-year-old Tyler, who was gunned down in his aunt's driveway next door to his own home in New Fairfield around 1 a.m. Thursday.
"It's something out of a Hollywood script," said John Hodge, the first selectman, or top elected official, in the town of nearly 14,000 people about 50 miles from New York City. He said he couldn't recall another killing in his eight years on the job.
State police spokesman Lt. J. Paul Vance said the boy had never been in trouble with the law, and some of those who knew him described him as a good kid with an easygoing personality. Investigators and acquaintances said they were at a loss to explain what he was doing outside dressed all in black and carrying a weapon.
"Certainly, that is the major question we are trying to answer at this point," Vance said.
State police said the shooting happened after Jeffrey Giuliano got a call from his sister next door saying that someone might be trying to break into her home in their neighborhood of attractive colonial-style houses. Giuliano grabbed a handgun and went outside to investigate, troopers said.
He confronted someone in a ski mask and opened fire when the person came at him with something shiny in his hand, police said.
When police officers arrived, Tyler was lying dead in the driveway with a knife in his hand, and his father, in a T-shirt and shorts, was sitting on the grass. Detectives informed the elder Giuliano several hours later that he had shot his son, Vance said.
"All in all, it's a tragedy," Vance said.
Police were investigating whether the father's gun was registered.
No one answered the door at Giuliano's home or his sister's.
Tyler was a student at New Fairfield High School and a Civil Air Patrol cadet. Some of those who knew him said he enjoyed spending time with family and flying gliders and small planes. He was adopted by Giuliano and his wife a few years ago, friends said.
One classmate said many students were baffled by what happened.
"I just thought it was so weird when I heard because I knew Tyler, not very well, but he was just a sweet person and he always made everyone laugh. I met him in the chorus room, actually, and he just wasn't the type to do what happened," said Erin Pallas, 16. "So it didn't make sense to us. It doesn't make sense to the student body."
Brett Rasile, a 14-year-old friend, said he and Tyler were playing an online game called Minecraft while talking and laughing together via Skype until about 10 p.m. Wednesday, when Tyler said he had to go to bed. Brett said Tyler wasn't in any trouble that he knew of, and nothing seemed out of the ordinary.
"Same old Tyler. He was perfectly fine," Brett said. "He didn't really leave any evidence, any hints towards what he would do."
Alicia Roy, New Fairfield superintendent of schools, said the elder Giuliano grew up in the town, holds summer music and zoology camps for his students and plays guitar in a local rock band that raises money for charity. He is affectionately known as "Mr. G" around Meeting House Hill School.
"He was the teacher you requested in the fifth grade. He was a great teacher. All the kids loved him," said Rosemary Rasile, Brett's mother.
Brian Wyckoff, 17, said Mr. G "was always walking around with a smile on his face. He always says hi to everyone."
The high school stayed open late to provide grief counseling for students and parents.
"The community is deeply saddened, and our hearts go out to all the family members," Roy said.

The parasitic mite Varroa destructor is a major contributor to the recent mysterious death of honey bee (Apis mellifera) colonies. New research published in BioMed Central's open access journalGenome Biology finds that specific proteins, released by damaged larvae and in the antennae of adult honey bees, can drive hygienic behavior of the adults and promote the removal of infected larvae from the hive.
V. destructor sucks the blood (hemolymph) of larval and adult bees leaving them weakened and reducing the ability of their immune systems to fight off infections. Not that honey bees have strong immune systems in the first place since they have fewer immunity genes than solitary insects such as flies and moths. These tiny mites can also spread viral disease between hosts. This double onslaught is thought to be a significant contributor to Colony Collapse Disorder (CCD).
But all is not lost - honey bees have evolved a way to fight back: hygienic behavior where diseased or parasitized larvae are removed from their brood cells, and Varroa-sensitive hygienic behavior which they use to reduce the number of reproductive mites on remaining larvae.
To find exactly how bees respond to hive infections, researchers from Canada looked at the natural behavioral of bees in the presence of damaged larvae and compared this to protein differences in the larvae and adults. After scanning 1200 proteins the team found that several proteins, including LOC552009 (of unknown function but similar to ApoO), found in the antennae of adults were associated with both uncapping brood cells and the removal of larvae. Other proteins were involved in olfaction or in signal transduction, probably helping the adults find infected larvae amongst a brood.
In damaged larvae, transglutaminase, a protein involved in blood clotting, was upregulated, which appeared to be a key component in regulating the adult's behavior. Other proteins indicated adaptations to help fight infection, including chitin biosynthesis and immune responses.
Dr Leonard Foster from CHIBI at the University of British Columbia, who led this research said, "Bee keepers have previously focused on selecting bees with traits such as enhanced honey production, gentleness and winter survival. We have found a set of proteins which could be used to select colonies on their ability to resist Varroa mite infestation and can be used to find individuals with increased hygienic behavior. Given the increasing resistance of Varroa to available drugs this would provide a natural way of ensuring honey farming and potentially survival of the species."

CHICAGO (AP) ? Just like age-defying baby boomers, older folks have seen a surge in knee replacement surgeries, driven partly by a desire to stay active and by joint-damaging obesity.
The findings are in a study of more than 3 million Medicare patients, aged 65 and older, who got artificial knees from 1991 through 2010. Almost 10 percent of the operations were redos ? replacing worn-out artificial joints.
The number of initial knee-replacement surgeries each year on these older patients more than doubled during that time, rising to nearly 244,000 in 2010. Patients were in their mid-70s on average when they had surgery; that age edged up slightly during the study.
The aging population and rising numbers of Medicare enrollees contributed to the increase. But the per capita rate also increased, from about 3 surgeries per 10,000 enrollees in 1991 to 5 per 10,000 in 2010.
"There's a huge percentage of older adults who are living longer and want to be active," and knee replacement surgery is very effective, said lead author Dr. Peter Cram, an associate professor of internal medicine at the University of Iowa.
The study appears in Wednesday's Journal of the American Medical Association.
The pace of growth slowed in more recent years ? possibly because increasing numbers of younger adults have also been getting artificial knees, which typically last 15 to 20 years. The troubled economy may also have slowed demand for an operation that costs about $15,000, the study authors said.
About 600,000 knee replacement surgeries are done each year nationwide on adults of all ages, costing a total of $9 billion, the authors said. A journal editorial says measures are needed to control costs of these operations, noting that demand has been projected to rise to as many as almost 4 million knee operations annually by 2030.
Obese older patient getting their first operations accounted for almost 12 percent, up from 4 percent. The rate was about the same in those getting redos. Obesity takes a toll on joints and can contribute to arthritis, a leading reason for knee replacement surgery.
The average hospital stay dropped from almost eight days to 3 1/2 days, but many patients were sent to rehab centers, mostly outpatient centers in the most recent years. Readmission rates after first and second operations increased in recent years for reasons including infections and surgery complications.
The results suggest that shortening hospital stays doesn't always achieve the goal of reducing costs, since readmissions increased ? a trend seen with other types of surgeries and illnesses.
Mortgage rates, especially after the tax deduction most homeowners can take, are as close to zero as they have ever been.?Home prices, for the most part, are still affordable, but are beginning to appreciate.

A recent headline that garnered national attention declared it is 45 percent cheaper to buy than to rent, although the caveat was it depended on the metropolitan market.
Still, from San Francisco to Denver to Boston, the trend is clear. Rents have been rising much faster than home prices. Rapidly rising rents on one side, stacked against the mortgage deduction and chance of appreciation on the homeownership side, is closing the gap between the cost of renting and buying, if not eclipsing it completely, in many cities.

John: Lane, in some hot submarkets across the country, rents are rising 10 percent or more, while vacancies are falling to the lowest levels in more than a decade.?What is going on?
Lane:? Market dynamics are at work. It is supply and demand. There is a lack of supply for rental properties, while demand is rising rapidly. That?s the story pretty much across the country.
John: What?s driving the rental demand?
Lane: Part of it relates to housing. If you look at the national homeownership rate, it pretty much peaked in the mid-to-high 60s before the collapse in the housing market. Now, it has settled to maybe 62 percent, from 67 percent or 68 percent. That five percentage point shift represents millions of people who were homeowners and are now renters.
John: Is it still the American Dream to buy a home, or do the Millennials (those born in the late 1970s and early 1980s) prefer the freedom of being a renter?
Lane: During the worst of the housing crisis, there were a lot of headlines saying that the Millennials didn?t want to buy. TIME Magazinedeclared that the American Dream of owning a home was dead. Since then, surveys have found that Millenials are as eager to buy a home as the Baby Boomers or Gen Xers. Maybe more so. Problem is, even if the millenials want to buy, their finding it difficult to actually do it.
John: Why?
Lane: Part of it is the economic environment. Unemployment is still high. They might be saddled with a lot of student debt. It?s also harder to qualify for a mortgage. Unfortunately, not everyone who rents a home can afford a mortgage or qualify for a loan.
John: What should a renter do who wants to buy a home?
Lane: First, there seems to be a bit of a tautology out there that they can?t afford to buy a home. There are a lot of programs out there for first-time home buyers. And as renters, they don?t have to worry about selling a home for a loss, preventing them from moving up. Yes, underwriting standards are not as loose as they once were, but they have eased a bit.
If a renter has decided they want to explore buying a home, they should ask a Realtor or a friend for the name of a good loan officer or mortgage broker who can tell them what their options are. They might be surprised to find they have a few options as far as loan programs.
They might start putting money away for a downpayment. They might consider a multi-generational living arrangement with other family members ? parents, siblings, grandparents ? who can help out financially. Even if they don?t contribute financially, multi-generational living is growing in popularity and may have social benefits.
John: Should every renter strive to be a homeowner?
Lane: Someone should only be buying a home because they think it is in the right time in their life to do so. But there may be a lot of renters out there who think there is no way they can afford a mortgage payment when in reality they can.
If they want to buy, they can get a roommate to help pay the mortgage payment, or rent out a room. When I was in graduate school, my wife and I rented out a room in our house so we could afford the mortgage. I bet it costs twice as much to rent that room today, even though the house hasn?t doubled in value.
John: Given today?s market conditions, with historically low mortgage rates, home prices well below their peaks in most markets, while rents are on a tear, is it a no-brainer to buy a home if you can qualify?
Lane: There are a lot of benefits to owning a home that aren?t just financial. There are a lot of psychological advantages to being a homeowner that go beyond any financial gain.
Still, my prediction is that if you look 15 years in the future, most people who are 25 or 30 today and buy now will be financially better off than those who choose to keep renting. Not that there is anything wrong with renting. But over time, owning a home is often the biggest foundation for building wealth. The home they buy today might not be the last one they own. Maybe they won?t sell their first home, and instead will rent it out to someone else and become what I call a ?Collector.? People can end up collecting several homes through their life and build an investment property portfolio. The sooner you buy your first home, the sooner you can start to build wealth through real estate.

For the first time, a 'how-to' report gives designers and architects strategies to promote active living AND maximize safety
Designing or modifying buildings and communities to facilitate physical activity must include strategies to maximize safety. A new report released today, Active Design Supplement: Promoting Safety, by the Johns Hopkins Center for Injury Research and Policy, New York City Department of Health & Mental Hygiene's Built Environment and Healthy Housing Program, and the Society for Public Health Education (SOPHE) provides explicit guidelines for urban planners, architects, public health advocates, and others to consider when promoting active designs. Experts from New York City's departments of Transportation, Buildings, and Design and Construction, and the Mayor's Office of People with Disabilities also contributed to the report. It is the first time a publication has been produced to bridge the two disciplines of injury prevention and active design, and contains information for designers, architects, planners, public health professionals and engineers.
Active Design Supplement: Promoting Safety serves as a companion to the original The Active Design Guidelines: Promoting Physical Activity and Health in Design, published in 2010 by the New York City departments of Health & Mental Hygiene, Design and Construction, Transportation and City Planning to help professionals working in urban and building design to promote active environments. This new publication identifies injury prevention strategies that align with Active Design Guidelines. Topics addressed include playground equipment and surfaces, complete streets, crime prevention through environmental design, and stair safety features such as good lighting.
"Communities across the U.S. have begun to change their built environments to increase physical activity and reduce obesity," said report co-author Keshia Pollack, PhD, MPH, an associate professor with the Johns Hopkins Center for Injury Research and Policy, part of the Johns Hopkins Bloomberg School of Public Health. "Now, the designers and architects involved in these projects have an additional resource to help them further incorporate injury prevention into their work."
Drawing on existing studies and evidence-based best practices for maximizing safety, the publication's authors identified complementary strategies to promote active living and injury prevention, including 18 strategies for urban design and 9 strategies for building design. These strategies can be applied to create health-enhancing built environments that also help to reduce the risk of intentional and unintentional injuries.
"Injury is the leading cause of death for Americans ages 1-44 and affects people at home, at work, at school, on the road and during play," said co-author Andrea Gielen, ScD, ScM, director of the Johns Hopkins Center for Injury Research and Policy. "As characteristics of the built environment can affect the risk of injury, it's critical that the fields of injury prevention and active living continue to collaborate so that safety can be considered from the onset of planning and design."
For example, when planning trails and paths for pedestrians and bicyclists, designers should consider how older adults, people with physical limitations, families with strollers, and bicyclists can all safely use the space. Signage and pavement markings can be useful to maximize safety in such circumstances. In addition, separating each of these various road users, especially pedestrians and cyclists from cars as well as pedestrians from bicyclists, is an effective design strategy for maximizing safety. Opportunities for rooftop gardens and play spaces should also include structures that prevent the risk of falls, especially among children.
"This publication demonstrates the positive impacts of active design strategies on both active living and promoting safety," said Karen K. Lee, MD, MHSc, FPRCP, director of Built Environment and Healthy Housing at NYC's Department of Health & Mental Hygiene, and principal author of the 2010 Active Design Guidelines as well as the Supplement. "Many active design strategies have multiple benefits. For example, complete streets, streets that can accommodate safe walking, cycling, transit use and driving, and improved lighting can increase physical activity of all kinds and also contribute to preventing unintentional injuries and crime."
Another key takeaway of the report is that multiple active design strategies can often be enhanced simultaneously by a single injury prevention strategy; for example, improved timing of traffic signals can benefit pedestrians, bicyclists, and transit users.
"By incorporating injury prevention strategies into projects of all scales, design professionals can realize buildings and neighborhoods that seamlessly integrate more healthful and active living with attention to design excellence, sustainability, and safety," said Joseph Aliotta, president of the New York Chapter of the American Institute of Architects. "This Supplement should be used by all architects, designers, and building owners in concert with the Active Design Guidelines as both reference and resource."
The multidisciplinary collaboration of the publication's authors, representing multiple sectors of local government, an academic injury research center, and a nonprofit professional organization, is yet another example of how collective expertise and efforts are essential to promote both active living and safety.
"Health education specialists work to educate children and adults about the need to be physically active and avoid injury," noted Elaine Auld, MPH, MCHES, chief executive officer of SOPHE and a contributing editor to the report. "We also realize multiple partners and strategies are needed to accomplish our health goals. We are grateful to the U.S. Centers for Disease Control and Prevention's Division of Unintentional Injury Prevention and Control for supporting the development and dissemination of this publication."

Is your website easy to navigate?
Every time you explore your website, you must imagine you are visiting it for the first time and that you don?t know the products. In your honest opinion is it easy to find your way around the site? Can you find the delivery information easily or is it hidden away? Do you have a returns section so that doubtful customers have their questions answered?
It is essential you cover any and all potential questions. In a shop customers can wander over to the retail assistant and easily gain an answer. With an online shop, there is no one to ask, and emails take time, and it is likely your customer will go elsewhere if they can?t find the answer to their questions.
Not only that, you must make it obvious where they will find this information on your site. Ambiguously named menus and poor layout leads to frustration and customer drop offs. People have very little spare time to give so make sure you make it easy to get around your website with easy menus, well labelled, and well categorized so that it makes sense.
If you aren?t sure if your site is clear enough, look into user testing. A user tester can be set challenges by you, and as they complete their challenge, they will film their screen and narrate as they navigate around your site so you can understand if they are having trouble, or have some better suggestions. Challenges can be anything from find out the delivery costs to your address, or find a specific product, to sign up for our mailing list.

When is it practical refinancing your mortgage loan? A simple yes or no to answer whether or not your mortgage loan must be refinanced is not possible. Just think about your property investing business for an instant. Whenever you come across a deal, do you make a simple yes or no final choice about purchasing an investment property without having careful consideration? For the sake of your financial well-being, I hope you do not.
Just like any real estate investment deal, the dilemma of would it make sense to refinance is determined by a number of circumstances. And as with any real estate investing prospect, some of these loan refinancing scenarios are not very advisable. Quite a few deals involve a ?maybe yet only if? answer and, needless to say, some deals are absolute no-brainers.
Let?s take a look at a couple of these cases and I?ll give you my advice on how to deal with the predicament of whether refinancing your loan is a good idea or the opposite. To start with, let?s look at debt. Should you settle debts by adhering to conventional advice through refinancing at a lower rate? For me, this is a deal breaker. Don?t do it. You?re better off making use of other people?s money to clear out debt (i.e. your month to month revenue from tenants in your rental properties.)
Up next, to help you cover rehab or remodel costs, should you refinance a loan? Even though I am against for it on this particular scenario, I wouldn?t call this one a deal breaker. In today?s market, you shouldn?t have any problem purchasing real estate investment properties well beneath market value. This is exactly why your final offer price should include these repair costs.
The only actual way refinancing receives a slight nudge closer to a yes is it if will be your private residence, you intend on residing at the house for many more years, and you want to improve your standard of living. Nevertheless, it is still a very minor nudge towards yes. Don?t forget that, you still must consider all the numbers and costs before continuing to move forward with the refinance.
Finally, does it seem sensible to refinance a loan if the purpose is to reinvest in more cashflow properties? I would most certainly agree to refinance the loan on this one scenario. Then again ? yes there?s a catch ? this scenario only is smart in certain situations. Also, you need to do your homework much like any property investing deal.
Do your due diligence and ask yourself if the investment can pay you more than the cost of the refinance. So make sure your costing is correct. Will this remortgage not cost you money and rather make you money? My reply is a yes if the answer is yes.
You must steer clear of getting caught up in all the buzz about low refinance rates. It does not seem sensible to refinance your loans nine out of ten times. And the one occasion it does seem sensible, you must be using that extra income to acquire more cash flowing assets.
